  • insurance certificate;
  • insurance claim of the beneficiary (s) to receive an insurance payment;
  • a copy of the identity card of the insured person, with the exception of the death case of the insured person;
  • a copy of the loan agreement certified by the lender;
  • a copy of the loan repayment schedule certified by the lender;
  • a document issued by the authorized body and confirming the fact of the insured event and (or) the reasons and results of the events, which can be considered as insurance events that need to be investigated or recorded;
  • the original or notarized copy of the conclusion of the Medical and Social Expert Commission on the designation of disability to the insured person in case of disability;
  • a copy of the death certificate of the insured person in the event of his death;
  • medical certificate of cause of death; in case of death from an illness, an epicrisis and an extract from the medical history are also required;
  • if necessary, a notarized copy of the certificate of inheritance.
