What documents must be provided if an insured event occurs?

If the insured person is alive at the time, the insurance contract expires:

 

  • insurance certificate;
  • a statement in the prescribed form;
  • a document proving the identity of the insured person.

 

In case of death of the insured person during the insurance period:

 

  • insurance certificate;
  • statement in the prescribed form;
  • a certified copy of the death certificate of the insured person;
  • other documents necessary to determine the cause and nature of the insured event;
  • notarized copies of documents proving the identity of the recipient of insurance and, if necessary, a certificate of inheritance.
